Study: Low-Dose Aspirin’s Risks Outweigh Benefits

TUESDAY, May 14, 2019 (HealthDay News) — Let’s say you’re one of the millions of older adults who takes a low-dose aspirin religiously, in the belief that it will guard against heart disease and heart attacks.

Now, a new review suggests your risk of a brain bleed outweighs any heart benefit that a daily aspirin might bring you.

Researchers said the findings support a recent change to guidelines on low-dose aspirin: The blood thinner should now be reserved for people at high risk of heart attack or stroke.

Others can skip it.

The change was issued in March by the American College of Cardiology (ACC) and the American Heart Association (AHA). The groups said that while the bleeding risk with aspirin has always been known, it now appears the risk is not worth it for most people.

Instead, the average person should focus on controlling their blood pressure, blood sugar and cholesterol, eating a healthy diet, getting regular exercise and not smoking.

“All of those things are more important than taking low-dose aspirin in preventing future heart attacks and strokes,” said Dr. Meng Lee, one of the authors of the new report.

“Our findings do support the latest change to the ACC/AHA guidelines,” said Lee, of Chang Gung University College of Medicine, in Taiwan.

For the study, the investigators pooled the results from 13 clinical trials testing low-dose aspirin in older adults with no history of heart problems or stroke. On average, aspirin raised the risk of bleeding in or around the brain by 37%, the findings showed.

The risk was still small: The researchers estimate that a daily aspirin would cause an additional two brain bleeds for every 1,000 people.

But for people at lower risk of heart attack or stroke, that’s a chance they probably should not take, according to the new guidelines.

And, based on two trials, people of Asian ethnicity might be at particular risk of brain bleeding. Patients in those studies saw their risk rise by 84%.

It’s not clear why, according to Lee — but other studies have found the same pattern.


The latest finding was published online May 13 in JAMA Neurology.

If it has long been known that aspirin carries a bleeding risk, why is the advice changing now?

Research in recent years has shown that the balance of risks versus benefits has changed, explained Dr. Eugene Yang, a member of the ACC’s Prevention Section and Leadership Council.

Earlier studies did suggest that the bleeding risks with aspirin were generally outweighed by its ability to curb the odds of a first-time heart attack and stroke.

But things are different today, Yang explained. People are smoking less and there have been improvements in controlling high blood pressure and cholesterol. That means for lower-risk people, the heart benefit of aspirin has diminished — making the bleeding risk more of a concern.

Yang stressed, however, that the guideline change applies only to people without “overt” cardiovascular disease. For people with a history of heart attack or stroke, or significant narrowing in the arteries supplying the heart, brain or legs, the advice stays the same.

“In those cases, you’re trying to prevent further complications,” said Yang, who is also a clinical associate professor of medicine at the University of Washington, in Seattle.

In addition, he pointed out, aspirin is not an absolute “no” for preventing first-time complications, either.

The guidelines say people over age 70 should avoid aspirin if they do not have overt cardiovascular disease. But it may still be considered for certain people ages 40 to 70 who are at heightened risk of cardiovascular complications.

“It’s not a simple, black-and-white decision,” Yang said.

If you are currently taking aspirin and wondering if you should stop, talk to your doctor first, Yang advised.

“There could be other reasons it was prescribed, such as lowering the risk of colon cancer or to prevent blood clots,” he said.

The Benefits of an Anti-Inflammatory Diet

TUESDAY, April 23, 2019 — The body’s immune system is designed to fight off threats, like infection-causing germs, through a process called inflammation. But a steady state of inflammation can lead to everything from diabetes to autoimmune diseases to heart disease to cancer.

Many of these health threats don’t come from foreign invaders like scary bacteria, but from some everyday foods you’re probably eating. Removing inflammation-provoking foods from your diet, or at least limiting them, can help you do a better job of protecting your well-being.

For many of the most common chronic diseases spurred by inflammation, the starting point is obesity. And obesity is often the result of overeating foods that foster inflammation. Most — though not all — of the foods that cause inflammation deliver minimal nutrition. Processed foods made with refined white flour and white sugar are top culprits. These include packaged white breads and rolls, baked goods and candy. Soda and other sugar-sweetened drinks are in the same category.

Fried foods and fats — such as margarine, shortening and lard — also promote inflammation. So do processed red meats, like bacon, sausages, hot dogs, salami and other deli meats. Even lean red meat should be limited to once or twice a week.

Replace these foods with those known to be anti-inflammatory, those high in natural antioxidants and other protective compounds.

These include good-for-you fats such as mono- and poly-unsaturated plant-based oils, most types of nuts and seeds, and fish with omega-3 fatty acids such as salmon, tuna and sardines. Despite Big Heart Benefits, Far Too Many Skip Statins

SATURDAY, March 16, 2019 — People who’ve already had a heart attack or stroke can cut their odds for another one in half if they regularly take cholesterol-lowering statins.

Yet new research found that only about 6 percent of patients take these drugs as prescribed by their doctor.

“Very few patients were optimally compliant. We found that the less compliant you were, the worse you did,” said the study’s lead author, Heidi May, an epidemiologist from Intermountain Medical Center Heart Institute in Murray, Utah.

Why are so many people skipping this potentially lifesaving medication?

May said the study wasn’t designed to tease out exactly why people didn’t take their medicine as they should. But she said it’s possible that patients may not understand how helpful statins can be. Not only do they lower cholesterol, they also help reduce inflammation in the body, she said.

May said she doesn’t think it’s a lack of education, at least not initially.

“Our institution does educate when people are discharged, but patients are given a lot of information. This study emphasizes the need for continued education to take medications as prescribed,” she said.

Researchers also don’t think cost is an issue, because the medicines are now generic and relatively cheap — about $ 10 for a 90-day supply.

But concern about side effects may be an issue for some. If that’s one of your concerns, May said you should talk with your doctor.

“The good from statins far outweighs the bad. In very few people, statins may cause muscle aches, and doctors can check with a blood test whether or not statins are causing that. If they are, there are other statins you can try, or a lower dose,” she said.

Researchers also suspect that some patients wrongly believe that after a few years on statins, they don’t need them anymore, or they’re just not that important.

Dr. James Catanese, chief of cardiology at Northern Westchester Hospital in Mount Kisco, N.Y., said there are all sorts of reasons patients don’t take their medicines and they’re reluctant to tell their doctor when they don’t. But he emphasized that taking these drugs as prescribed is vital.

“Taking statins is really going to reduce the chance of having another heart attack or stroke dramatically,” said Catanese, who wasn’t involved with the research.

May’s study included nearly 5,500 people who were diagnosed with atherosclerotic cardiovascular disease. That means they had plaque buildup in their blood vessels. That buildup can cause a number of problems, such as heart disease, stroke and peripheral artery disease (PAD).

Nearly 80 percent of study participants were male. Their average age was 57 years.

When researchers looked to see who was taking a statin as prescribed, they saw that 25 percent never filled their first prescription. And 25 percent didn’t fill their second one.

Just 6.4 percent took the drugs as prescribed 80 percent of the time or more. But those who took the drugs the most had a 48 percent lower risk of having another heart event or stroke.

“The better you are at taking your medications, the better you’ll do,” May said. “Remember, you might feel fine right now, but the statin is preventing you from having a problem a few years down the road.”

Catanese suggested developing a habit around taking your medications.

“It doesn’t matter when you take a statin, so take it with other medications. Put the bottle on the breakfast table and turn it upside down when you take it. Or put an alarm on your phone. You need to form a habit around taking your drugs, because it’s not OK to only take them once in a while. It needs to be daily,” he advised.

The study is scheduled to be presented March 16 at the annual American College of Cardiology meeting, in New Orleans. Findings presented at meetings are typically viewed as preliminary until they’ve been published in a peer-reviewed journal.
